NDC Stands for National Drug Code, According to the Drug Listing act 1972 for all the drugs there is unique product identifier used for the drug indent for Human use. NDC Number Format: 10 Digit numeric  First 4 – 5 digit represents the following Labeler Code Second 3 – 4 digit Product Code   Third 1 – 2 Package Code NDC numbers are grouped under the following format 4-4-2 5-4-1 5-3-2 But CMS has created 11 digits NDC, NDC with leading zeros so the format will be 5-4-2

What is CPT 69210?

Removal of cerumen, in simple words removal of ear wax, most of the insurance will not cover for ear wax removal, and all the insurance will deny the procedure stating it is included procedure with office visit.  following are the Medicare guidelines to bill Removal of cerumen procedure. ·          Cerumen removal is the only reason for the visit. ·          Cerumen removal is performed personally by a physician or midlevel provider. ·          The patient is symptomatic (pain, pressure, poor hearing, etc.) from excessive cerumen. ·          Cerumen removal requires more than drops, cotton swabs, and cerumen spoon. ·          Documentation in the patient record shows that the procedure required significant time and effort. What is CPT 20526, 28899, 64450 and 64455

All the above mentioned CPTs are related to Injections  Injection of a carpal tunnel may be indicated for the patient with carpal tunnel syndrome if oral agents and orthoses have failed or are contraindicated, or as adjunctive therapy to systemic agents for an inflammatory arthritis when those agents have not yet become effective and the patient experiences a relative entrapment syndrome manifested by moderate to severe pain. Note that this procedure has its own CPT code, 20526 . Injection of a tarsal tunnel is indicated for the patient with tarsal tunnel syndrome if oral agents have failed or are contraindicated, or as adjunctive therapy to systemic agents for an inflammatory arthritis when those agents have not yet become effective and the patient experiences a relative entrapment syndrome manifested by moderate to severe pain. Though there are many similarities between this and carpal tunnel syndrome, the best apparent CPT code for tarsal tunnel injection is 28899 . 64
